Each year, JC Mainstage requires updated forms from every participant. These forms ensure we can safely and effectively operate our productions.

Please download, complete, and print all required forms. Once filled out, return them directly to the director, Corey Snyder.

These forms are required annually, so even if you've filled them out in the past, you must submit new ones each season.
